• Procedure type

• Referring provider

• Payment details

This information is used solely to fulfill our merchant service duties and to provide transaction confirmations or assist with billing inquiries.

We do not sell, rent, or otherwise disclose personal information to unrelated third parties. All data is stored in HIPAA-aligned environments with strict access controls, and our team is trained on handling protected health and financial information.

Patients may contact us at any time to request a record of their transaction or clarification on how their information has been used.
